CASE

A young woman works in a medical laboratory. She had been married for 2 years during which

she got pregnant twice and aborted twice. Laboratory investigations revealed a parasitic cause.

GIVE REASONS

1. May develop due to spontaneous arrest of the disease or due to incomplete treatment with antimony compounds.

2. Total IgM is always elevated due to antigenic variation of the surface coat of the organisms.

3. In giardiasis, the parasite is not regularly excreted in stool.

4. Toxoplasmosis is asymptomatic in the great majority of cases. if it infects an immunocompromized patient, the patient develops acute disseminated toxoplasmosis that may end fatally.

5. P. falciparum infection is usually severe, fatal and causes malignant malaria. In this infection, parasitized cells develop knobs on their surface. So they adhere together and to specific receptors on the endothelial cells of blood capillaries of internal organs. This leads to partial occlusion of capillaries that may cause anoxia and necrosis which may be fatal. According to the site of occlusion of blood vessels, the clinical picture of falciparum malaria takes different forms.
