(stump of chronic inflammation)
Definition: A chronic specific inflammation of nasal mucosa
Aetiology:
rhinosclerhoma is caused by infection of Klebsiella rhinoscleromatis.
Pathology:
Site: nose is the most common site but larynx, pharynx and upper trachea can be affected
Macro(gross): single or multiple nodular swelling filling the nasal cavity and can extend to upper lips, pharynx,larynx and upper trachea
Micro:
Nasal mucosa(covering): may show squamous metaplasia due to chronic irritation
Submucosa( core): infiltrated by chronic inflammatory cells, progressive fibroisis and endarteritis obliterans. The chronic inflammatory cells are:
1) Mikulicz cells(pathonomonic): macrophages with clear cytoplasm and central or eccentric nucleus due to hydropic degeneration
2) Plasma cells and russel bodies
3) Lymphocytes
Complications:
1) Nasal obstruction
2) Deformity
3) Epistaxis
4) Secondary bacterial infection
5) Ulceration
